Client PORTRAIT AGREEMENT:

in order to become a client of Tracy Joy Photography)
I, ______________________________ (Subject in form below),
understand and give permission for Tracy Tignino of “Tracy Joy Photography” (Photographer) unrestricted right and permission to copyright, use, or publish photographic portraits of myself (or minors listed) for studio display, blogging publication, advertising, professional print competition, or other lawful purposes, ie: (in other words, use on my blog, website, portfolio, studio, etc). The names of minors will NEVER be used in full on any of my sites, they will be listed as a first and last initial (ie: TA). I protect and adore all of my clients, esp the minors, and do all that is right and lawful to protect their rights.
- I hereby waive any right that I may have to inspect and approve the finished product that may be used in
connection with an image that the Photographer has taken of me, or the use to which it may be applied. - I acknowledge that my dealings with Tracy Joy Tignino, have been conducted in a completely proper and highly professional manner, and this release was willingly signed.
- I understand that the Photographer maintains copyright to these images, and copy or use without written permission from said photographer is violation of copyright laws
- I certify that I am above the age of 18 years old and not a minor, and am free and able to give such consent for myself, and/or my minor children.
By filling out the below form, this means that you have:
1) Read & agree, with the above statement & terms
2) Understand that filling out the below form is your “cyber signature” and agreement to the above statements & terms.
3) Are ready to become a client of Tracy Joy Photography!
